The Central Surgery in the Oadby and Wigston area is seeking a Clinical Pharmacist with independent prescriber status to support general practice prescribing and medicines optimisation. You will work in a collaborative team, focusing on long-term condition management and improving patient outcomes.

Experience in diabetes care, QOF, and medicines reconciliation is desirable. The role offers a supportive NHS pension scheme and negotiable hours, with on-site work at the practice.

Qualificações

• Masters degree in pharmacy or equivalent and registered with GPhC.
• Independent prescriber status with experience in a General Practice.
• Experience applying medicines optimisation and QOF in primary care.

Responsabilidades

• Manage same-day medication queries and review acute prescription requests.
• Conduct medicines reconciliation on discharge and clinic letters.
• Lead long-term condition clinics with focus on diabetes management and optimisation.
• Signpost to community pharmacy and collaborate with GP colleagues as needed.
• Provide medicines information and support to practice staff and patients.
