Qualifications

• Maintain effective relationships with patients, relatives and carers.
• Collaborate with multidisciplinary team to deliver care on an older adult ward.
• Assist with activities of daily living and personal care under supervision.

Responsibilities

• Implement and evaluate individual care plans under supervision of a nurse.
• Record results in the appropriate format and report as required.
• Support management of patients including handling challenging behaviour with de‑escalation techniques.
