Job summary

Swansea Bay University Health Board is inviting applications for a role within the Pulmonary Rehabilitation service. The post involves working across the Swansea Bay UHB area to support service delivery, in outpatient clinics, community groups and in patients’ homes when needed.

You will work as part of the Occupational Therapy/Physiotherapy Team, undertaking intervention programmes for individuals or groups and supporting therapists with clients who have complex needs in diverse settings

Qualifications

  • Background in health or allied health fields preferred.
  • Experience in rehabilitation or clinical setting is advantageous.

Responsibilities

  • Travel across the Swansea Bay UHB area to support service delivery.
  • Work within outpatient clinics, community groups and patients' homes when necessary.
  • Assist physiotherapy, occupational therapy and nursing teams with client-centred rehabilitation.
  • Undertake intervention programmes with individuals or groups.
  • Maintain clinical records (paper and electronic) and report accurately.
  • Carry out administrative duties to support clinical and departmental needs.

Job description

The Pulmonary Rehabilitation team includes Physiotherapy, Occupational Therapy, Dietetics and respiratory Nursing. The service supports people presenting with chronic respiratory disease. The team work closely with the Adferiad Service . Your job will involve both clinical and administrative duties to facilitate the service.

The post holder will be required to travel/undertake clinical work across the Swansea Bay UHB area to support service delivery working directly and indirectly with patients and families in outpatient clinics/ community groups and in their own homes when necessary and staff members., Be a fully engaged member of the Occupational Therapy/Physiotherapy Team providing a service within a defined area, supporting the therapists with clients who may have complex needs in a range of settings including hospital, community, patient's homes, schools, and out-patient settings

Undertake intervention programmes with individual clients or groups this may involve fitting and instruction on the use of adaptive equipment and mobility aids the supervision of adaptive daily living skills and leading therapeutic groups.

Supervise, monitor, and encourage patients undertaking rehabilitation programmes, promoting independence and engagement in rehabilitation.

Maintain clinical records (paper & electronic) and report accurately to others.

Carry out administrative duties in relation to clinical, service, departmental needs

Work flexibly to meet the needs of the service, including 7-day shift pattern, with extended working hours covering 365 days of the year
