We are seeking a dedicated and experienced GeneralPractitioner to lead our Special Allocations Scheme (SAS) a vitalservice providing care to patients removed from mainstream GP surgeries due tochallenging behaviours.
This role offers a unique opportunity to deliver compassionate,holistic care to complex patients in a supportive, multidisciplinaryenvironment. The position includes a mix of remote triage and face-to-faceconsultations, with flexibility to suit your working preferences.
Must be available Monday or Thursday for daytime hours

Patient Care: provide first-contact care for patients with undifferentiatedphysical and psychological health problems, using their skills inhistory-taking, physical examination, and clinical decision-making toestablish diagnoses and management plans. This includes provision of care foremergency conditions (and appropriate management) and ensuring chronic healthneeds are met.
Continuity of Care: ensuring continuity of care by maintaining ongoing relationships withpatients, managing chronic conditions, and coordinating care with otherhealthcare providers.
Preventive Care: provide preventive care, including health education,vaccinations, and screening programs to prevent illness and promote health.
Best practice : ensuring appropriate prescribing individualised to the patient withinthe scope of primary care. Working in line with current up to dateguidelines. Requesting appropriate investigations and following up on resultsas they are received
Holistic Approach: take a holistic approach to patient care, consideringphysical, psychological, and social factors when diagnosing and treatingpatients.
CulturalCompetence:Understanding and respecting cultural differences in healthcare beliefs andpractices is important for providing patient-centred care.
Referral and Coordination: recognising when to refer patients to specialists andcoordinate with other healthcare professionals to ensure comprehensive care.
Patient Advocacy: act as advocates for patients, ensuring they receiveappropriate care and support within the healthcare system. Supporting withallied professionals involved including MDTs and Best Interest meetings
Record Keeping: Maintaining accurate and up-to-date medical records for allpatients, keeping records confidential, and ensuring adherence to GDPR
Professional Development: engage in continuing professional development to keepup-to-date with medical advancements and maintain their competence.
Clinical Governance: participate in clinical governance activities, such as auditsand quality improvement initiatives, to enhance patient care quality.
Practice Management: support clinical and non clinical staff and facilitate supportwith administration of the practice and attending practice meeting. Onoccasion working with wide teams within the organisation
