An exciting opportunity has arisen to join our multi-disciplinary PCN team.
We are a well- established PCN team who are passionate about developing and delivering excellent quality local services to meet the needs of our patients. Our practices are keen to work with our team of Clinical Pharmacists, ANP, FCPs, Care coordinators, Social Prescribing Link Workers, Health and Wellbeing Coach and Mental Health Practitioner.
Due to the new permanent addition in the ARRS Scheme to introduce Newly Qualified General Practitioners we are looking to expand our team by seeking to employ a newly qualified General Practitioner to work alongside our practices and PCN staff.
To perform as an autonomous general practitioner working independently and with other health care professionals to assess, diagnose and treat the conditions of patients attending within a primary care setting.The successful candidate will rotate through each of the PCN practices, spending dedicated blocks of time in each, allowing for full integration into the teams and the opportunity to understand the varied operational dynamics of each practice. As a newly or recently qualified GP, you will benefit from daily support from experienced GP colleagues
The role involves both clinical and administrative work which are based on the EMIS clinical system, which is used by all three practices, balancing direct patient care with involvement in broader PCN initiatives.
Provide expert professional advice to patients, carers and colleagues and ensure the maintenance of clinical excellence.
To be professionally accountable and responsible for all elements of clinical practice in accordance with the GMC code of conduct ensuring the safety of patients and quality of their care is primary.
The post holder will need to be capable of independent travel across the Eastbourne area and work flexibly to meet the needs of the Business.
This role is a permanent ARRS position.
Eastbourne East PCN is made up of three GP Practices with a weighted list size of 37,575 spread across a fairly compact urban area. We work closely, collaboratively to deliver care to our patient population, supported by e3healthcare Ltd our own exclusive limited liability company. Princes Park Health Centre and Stone Cross Surgery are both training practices and will therefore offer comprehensive support to newly qualified GPs.
The PCN formed in 2019 with no changes to its original members to date.
Our three core PCN Practices are:
To undertake a variety of duties including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing/authorising repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion.
Manage a broad range of acute and chronic illnesses, ensuring that care is delivered in line with evidence-based guidelines and best practices.
Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation.
Assessing the health care needs of patients with undifferentiated and undiagnosed problems
Screening patients for disease risk factors and early signs of illness
Participate in the management of chronic diseases, such as diabetes, hypertension, asthma, and COPD, ensuring that patients receive optimal long-term care.
In consultation with patients and in line with current practice disease management protocols, developing care plans for health
Providing counselling and health education
Recording clear and contemporaneous consultation notes to agreed standards
Collecting data for audit purposes
Compiling and issuing computer-generated acute and repeat prescriptions
Prescribing in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriate
The post-holder will be required to visit patients during surgery time in an emergency if clinically indicated.
The post-holder will undertake a share of home visits per week as allocated.
The post-holder will also be required to work a share of the PCN Enhanced Access Service, via a rota system.
