Our practice is a bustling community of medical professionals, comprising 2 GP Partners, 3 Salaried GPs, 2 Advanced Clinical Practitioners, 1 Clinical Practitioner, 3 Practice Nurses, 2 Physician Associates, 2 Health and Wellbeing Coaches, 3 Care Coordinators, 3 HCAs, 2 Clinical Pharmacists, 1 Practice Manager, 4 Administrators, 1 Reception Manager, and 9 Receptionists, as well as members from our local community services.

Main duties of the job

We are looking for candidates with extensive experience in several key areas. These include:

Job responsibilities

The following are the core responsibilities of the practice nurse. Other tasks may be required on occasion, depending on factors such as workload and staffing levels.

Clinical

  1. To assess, plan, implement and evaluate patient care
  2. To be involved in immunisation, including campaigns as required by the public health needs of the Practice population
  3. To exercise knowledge and skills in chronic disease, wound assessment and management, ear care, contraception, and other clinical skills as appropriate to the role
  4. To participate in infection control
  5. To participate in the screening and health promotion activities of the Practice
  6. To develop skills in sexual health, including cervical cytology and sexual health screening
  7. To participate in chronic disease management within the Practice guidelines
  8. To make referrals to, and receive referrals from, other health care professionals
  9. To demonstrate a level of knowledge and clinical skills appropriate to the needs of the role
  10. To prescribe medications within competence and the scope of Practice

Management

  1. To participate in the development of Practice philosophy, strategy, and policy
  2. To be involved in the management of change within the Practice and the implementation of systems to improve patient care
  3. To participate in and contribute to the clinical governance agenda of the Practice
  4. To assist the Practice in achieving the Quality and Outcome Framework (GMS contract), Practice Delivery Agreement (PDA), Investment and Impact Fund (IIF) and other local and directed enhanced services.
  5. To evaluate and audit care
  6. To participate in multidisciplinary protocol development
  7. To be responsible for the maintenance of supplies and equipment

Training and education

  1. To contribute to the training of Medical and Nursing students, and other students and members of staff as required
  2. To participate in the clinical meetings within the Practice
  3. To maintain one's professional development and PDP
  4. To attend study days, courses and other educational events as required by the role
  5. To participate in the Practice appraisal process

Communication

  1. To maintain accurate and contemporaneous records
  2. To work with the broader nursing team in developing an integrated service for patients
  3. Make referrals to other Practice team members and voluntary and statutory agencies as necessary.

Professional

  1. To work within the NMC Code of Professional Conduct and Scope of Professional Practice
  2. To undertake professional development as required by the role
  3. To undertake an annual non-medical prescribing audit.
Person Specification
Experience
  • Experience of working within the Primary Care Environment
  • Up-to-date training in Childhood and Adult Immunisations
  • Training and experience in the management of long-term conditions
  • Training and experience in contraception and sexual health including cytology
  • 2 years post-registration experience
  • Excellent written and oral communication skills
  • Ability to prioritise and manage own workload
  • Ability to work well as a member of a multi-disciplinary team
  • Well-developed problem-solving skills
  • Motivational interviewing skills
