An insightful opportunity for an Experienced Clinical Pharmacist to join the Poplar & Limehouse Health & Wellbeing Network in London. Job summary
Experienced Clinical Pharmacist
You would be joining an existing team of Network Clinical Pharmacists, based in the heart of the East End at the Aberfeldy Practice.
The Practice's mission is to provide accessible, compassionate, and high-quality healthcare to every patient they serve. They are committed to promoting health and well-being, preventing illness, and delivering personalised care that meets the diverse needs of the community.
As part of the pharmacy team you will be responsible for implementing effective medicine management through the identification of areas of improvement and innovation. We see these roles as instigators of a dynamic medicines management approach, including delivery of training and advice and expertise to other Primary Care colleagues, thus helping to maximise cost-effective prescribing, whilst improving the quality of patient care.

Main duties of the jobSuccessful candidate will:
Be a qualified pharmacist (ideally having completed CPPE pathway and an Independent Prescriber course)
Have a passion for providing high quality care
Work within the local community and adopt an integrated multidisciplinary approach to service delivery
Medication reviews
Interested in supporting long-term condition reviews for patients
Audits
Give leadership to the Medicine Optimisation Scheme and PCN initiatives
Review hospital discharge summary and add, amend and remove medication as necessary
Review CAS alerts and ensure compliance for CQC
Attend practice meetings, MDTs, PLTs as necessary
A minimum of 5 years post-qualification experience is desirable!

Key responsibilities of this Post
1. Be able to work flexibly and supportively in a general practice setting alongside other clinicians, particularly existing Clinical Pharmacist colleagues
2. Develop and manage a medicines management plan and deliver patient services as determined by practice policy and local and national guidance.
3. Help build the medicines management approach in designated practices across the Network, including delivery of training and advice and expertise to other Primary Care colleagues, to maximise cost-effective prescribing and improve the quality of patient care.
4. To identify, develop and promote quality improvements in medicines management and pharmaceutical understanding across community pharmacy & GP practices ensuring that best practice is adopted.
5. To develop and support new ways of working and workflow redesign linked to the safe, effective, high quality, use of medicines. by providing professional specialist medicines management
