Central Cheshire Integrated Care Partnership (CCICP) is expanding its team of General Nursing Assistants (GNAs) who work as part of the Integrated Placement of Care Hub. GNAs provide care and support to patients at home, including assisting with personal care, medication administration, assisting with light meals, support with catheters or stomas, and rehabilitation exercises. CCICP operates across five care community hubs: Nantwich, Northwich, Winsford, Crewe and SMASH (Sandbach, Middlewich, Alsager, Scholar Green and Haslington).
As a General Nursing Assistant (GNA), you will work under the direct supervision of the nursing team as part of the IPOCH – with a home-first focus on rehabilitation and promoting independence for patients leaving acute/bed-based settings or requiring step-up care in their own homes. The role includes providing end-of-life care in patients\' own homes, with a dignified and high-quality approach for patients and families during challenging times.
You will receive dedicated GNA training, with a Band 6 nurse acting as your mentor for appraisals and one-to-one meetings. You will also have day-to-day support from senior GNA practitioners.
