Job summary
Oxford Health NHS Foundation Trust seeks a Mental Health Practitioner for the Gateway Team in Aylesbury. Qualifications
- Experience in adult mental health settings.
- Ability to travel between sites within the Trust.
- DBS check required as part of onboarding.
Responsibilities
- Serve as the main point of contact within the Gateway, making clinical decisions regarding referrals and providing advice and support to professionals, service users and families.
- Perform comprehensive triage with risk assessment for new referrals.
- Provide specialist advice to referrers and liaise with service users to establish current presentation and safety plans while awaiting assessment.
- Support delivery of link worker provision into primary and secondary care and to staff in partner agencies with consultation and training.
- Assist the Gateway/Team Operational Lead to deliver the service meeting national guidance and local KPIs; support learning and development of new team members.
