Duties and Responsibilities
Patient Support

- Consultation Skills: Utilize your consultation skills to collaborate with patients, ensuring they use their medications effectively.
Care Coordination
- Care Home Support: Facilitate medication reviews and reconciliation for new care home residents, and synchronize medicines for patients transitioning between care settings in collaboration with local community pharmacies.
- Support Clinical Pharmacists: Assist in organizing necessary monitoring tests for Structured Medication Reviews (SMR).
Public Health and Social Care
- Expert Advice: Provide valuable public health and social care advice, including lifestyle guidance and service information, helping address local health inequalities.
- Shared Care Protocols: Manage shared care protocols and work with Clinical Pharmacists to handle more complex patient cases.
Service Delivery
- Commissioned Services: Support the Primary Care Network (PCN) in delivering on Quality and Outcomes Framework (QOF), Impact and Investment Fund (IIF), and other locally commissioned enhanced services.
