Job Summary

An exciting opportunity awaits a highly motivated and experienced Lead Pharmacist to join the General Practice Team at Valentine Health Partnership. This role will work closely with the GP clinical lead to spearhead medicines optimization plans and integrate pharmacy professional practices into daily patient care, aiming to improve health outcomes and reduce inequalities within our diverse communities.

Key Responsibilities
  • Leadership and Strategy: Develop and deliver the VHP medicines optimization plan in collaboration with the GP clinical lead.
  • Clinical Guidance: Provide specialist pharmaceutical advice and clear leadership on effective medicine strategies.
  • Program Development: Support and advise on pathway development, transformation, contracting, and resource use.
  • Collaboration: Build and maintain relationships with SEL ICB, South East London system, and other stakeholders.
  • Performance Monitoring: Develop and implement measures to assess performance against medicines optimization workplans.
  • Risk Management: Manage clinical and financial risk strategies for medicines optimization.
  • Workforce Development: Lead initiatives to develop the pharmacy workforce at VHP, including supporting PCN Pharmacists and Community Pharmacy Neighbourhood Leads.

Qualifications and Experience

Essential:

  • Qualified Clinical Pharmacist registered with the General Pharmaceutical Council
  • Clinical diploma or equivalent
  • Independent prescriber

Desirable:

  • Membership in the Royal Pharmaceutical Society
  • Experience working with colleagues from diverse professional backgrounds
  • Knowledge of read-codes/clinical coding
  • Master’s degree in Pharmacy (MPharm) or equivalent
