Summary:

As a Community Hospital Pharmacist, you will play a pivotal role in providing clinical pharmacist support to GP practices within North Hammersmith & Fulham PCN. Under the guidance of the senior pharmacy team, your responsibilities include offering pharmaceutical support to healthcare professionals (HCPs) and patients within the PCN. Conducting face-to-face structured medication reviews for patients in various settings, including clinics, practices, patients’ homes, care homes, or bedded services, is essential. Initially, you’ll work under supervision until assessed as clinically competent to conduct SMRs independently. Additionally, you’ll support long-term chronic disease management through polypharmacy review or medicines optimization prescribing clinics. Utilizing clinical tools, you’ll proactively identify patients for structured medication reviews, such as those who are frail, recently discharged, or have a history of falls.
