Rural Warwickshire North Primary Care Network (PCN) is seeking a proactive and skilled Clinical Pharmacist to enhance our medicines management services. This is an exciting opportunity to work closely with our team and contribute to the health and well-being of our patients.

Job Summary

As a Clinical Pharmacist, you will play a pivotal role in ensuring the effective, efficient, and safe delivery of medicines management services. You will collaborate with our experienced team of pharmacists, receive ongoing support to expand your professional competencies, and work within a friendly and nurturing environment.

Key Responsibilities
  • Medication Reviews: Conduct face-to-face, remote, and e-consultations, managing patients, including those with complex polypharmacy. This involves patient education, prescribing, and monitoring in accordance with guidelines and professional standards.
  • Team Collaboration: Work closely with the broader healthcare team, providing advice and support on medicine management issues.
  • Prescribing Audits: Conduct and participate in prescribing audits to ensure compliance with guidelines.
  • Cost-Effective Prescribing: Promote and encourage cost-effective prescribing practices within the PCN.
  • Drug Safety: Adhere to and enhance PCN systems for drug safety and monitoring, including conducting regular safety audits.
  • Knowledge Sharing: Actively share knowledge and best practices with the wider team and participate in PCN meetings and external events.
  • Target Achievement: Contribute to meeting high targets for the Investment and Impact Fund (IIF) and the Quality and Outcomes Framework (QOF).
  • Patient Care: Perform medication reviews for housebound or care home patients as required and support PCN initiatives.
