Qualifikationen

• Screen referrals and undertake specialist assessments
• Provide brief follow-up or facilitate alternative services
• Coordinate multidisciplinary response to crises
• Assess, plan, implement and evaluate care with service users and carers

Aufgaben

• Lead a caseload of patients and be at the forefront of care in collaboration with the Operational Team Manager, Clinical Leads/Senior Practitioners, and Inpatient/Home Treatment teams
• Assess health and social care needs and negotiate care plans with service users and their carers
• Proactively undertake rigorous risk assessment and risk management strategies
• Apply specialist clinical knowledge to carry out evidence-based interventions and supervise junior staff as required
