A community healthcare provider in Brighton is seeking a motivated administrative support role within the Frailty & Outreach Service. The successful candidate will manage patient records, support long-term condition reviews, and assist clinical staff. Ideal for someone organized with strong IT skills and a passion for patient care. This position offers a full-time or part-time contract, with a focus on improving healthcare outcomes in the community.
The Frailty & Outreach Service is a proactive, community-based team within the PCN, working to improve health outcomes for adults living with frailty or long-term conditions. We bring together nursing staff, allied health professionals, and support staff to provide holistic, joined-up care.
The post holder will be an integral member of the Frailty & Outreach Service, combining administrative expertise with direct clinical support. A key part of the role is contributing to the delivery of LTC provision for housebound patients through accurate management of patient spreadsheets, timely reporting of review outcomes, and proactive liaison with GP surgeries to support smooth service delivery.

East & Central Brighton Primary Care Network (PCN) is a collaboration of nine GP practices: Arch Healthcare, Ardingly Court Surgery, Broadway Surgery, Park Crescent Health Centre, Pavilion Surgery, Regency Surgery, St Peters Health Centre, Wellsbourne Healthcare CIC, and Woodingdean Medical Centre. Together, these practices serve a diverse population across Brighton, working collectively to strengthen local healthcare provision and improve patient outcomes. The PCN delivers extended services beyond core GP provision, including proactive frailty and long-term condition management, health outreach, and additional patient-facing initiatives designed to meet the needs of the community. Collaboration within the PCN enables teams to share knowledge, streamline pathways, and provide integrated care for patients with complex needs.
