Job summary

Gateshead Health NHS Foundation Trust in the North East of England is seeking an experienced Frailty Nurse Practitioner to join the Frailty Service, rotating between the Acute Frailty Team and the Frailty Virtual Ward.

You will deliver advanced clinical assessment, non-medical prescribing and care planning for people living with frailty across acute and community settings, working with patients, families and the multidisciplinary team to promote independence and safer, closer-to-home care.

Qualifications

• Registered General Nurse with current NMC registration and post-registration experience.
• Experience delivering care to frailty populations is desirable.
• Ability to work across acute and community settings within a multidisciplinary team.

Responsibilities

• Provide senior clinical leadership in frailty care and service development.
• Manage a defined caseload of frailty patients with complex needs.
• Deliver high-quality, patient-centred assessments and management plans.
• Support safe transitions between acute, community and social care settings.
