We are looking for an experienced nurse, who has experience in managing a team and who has sound clinical knowledge of older persons care, with a specific focus on frailty.
You should be a committed inspirational leader, with excellent communication skills and who is enthusiastic and embraces innovation.
You will work collaboratively across the acute setting within the hospital with the aim of enabling supporting the current Frailty In reach servicewhich supports admission avoidance.
Within this role the candidate will work alongside the geriatricians to provide:o An early recognition of a frail and elderly patient in the acute trust settingo To support specialised review of clinical condition in conjunction with consultanto Enable MDT working across disciplines to facilitate safe dischargeso Support and promote service developmento Be instrumental in the development and delivery of education and training programmes that enhances care staffs' knowledge to Frailty
You will be a pioneering practitioner who is clinically sound with up-to date knowledge in this specialist environment providing the highest standardof individualised care to patients. You will be or have evidence of working towards an advanced care practitioner level of nursing.
