We are looking for an experienced and forward-thinking General Practitioner (GP) to join our pioneering Virtual Frailty Ward team in Scarborough. This innovative service supports older adults living with frailty to receive timely, hospital-level care at home.

The role involves providing expert clinical input to a multidisciplinary team, helping to optimise patient care, reduce avoidable hospital admissions, and promote independence. Key responsibilities include:

  • Providing senior clinical leadership and decision-making within the Virtual Frailty Ward
  • Undertaking assessments of acutely unwell or deteriorating frail patients
  • Developing and implementing holistic, person-centred care plans, including advanced care planning where appropriate
  • Liaising closely with consultants, ACPs, community nurses, therapists, pharmacists, and social care professionals
  • Prescribing, monitoring, and reviewing medications in line with best practice and patient needs
  • Contributing to service evaluation, quality improvement initiatives, and education for colleagues and trainees
  • Participating in rota cover, including virtual ward rounds and urgent clinical reviews

We are seeking a collaborative and compassionate GP who is passionate about improving outcomes for older adults with frailty. The ideal candidate will have an interest in frailty, urgent care, or community-based integrated services.
