This opportunity involves joining a multidisciplinary frailty team that provides autonomous clinical support for patients undergoing frailty health assessments, including diagnosis, treatment, and discharge within their competence.
The Advanced Practitioner (AP) will deliver expert care and treatment to patients within established protocols and guidelines, with the authority to make decisions outside these frameworks when necessary, based on best practice.
The role requires rotation across various sites including Stoke Mandeville Hospital, Frailty Same Day Emergency Care (FSDEC), hospital at home team, community inpatient areas at Amersham and Buckinghamshire, and Wycombe General Hospital’s day unit. The post involves working flexibly to support a developing 24-hour frailty care model.
Main duties of the job
- Promote health and prevent illness through comprehensive patient assessments for risk factors and early signs of disease.
- Utilize a broad knowledge base to determine evidence-based interventions, including prescribing medication where permitted, and monitor their effectiveness.
- Plan and manage complete episodes of care, working collaboratively, delegating, and referring appropriately to optimize outcomes and resource use.
- Support patients in managing their health and daily lives.
- Contribute to practice development programs within the Frailty Team and Department of Medicine for Older People.

Person Specification
Education, Qualifications & Training
- First-level degree (HEI Level 5 or 6) or equivalent in the primary profession
- NMC or HCPC registration
- Masters in Advanced Practice or equivalent, covering the four pillars
- Qualified as a non-medical prescriber (if applicable)
- Evidence of ongoing professional development, including Level 7 modules
- Willingness to work towards accreditation as an Advanced Practitioner, including NHSE digital badge
- Postgraduate management/leadership training
- Teaching qualification
Experience
- Substantial post-registration experience, including three years at band 7 or equivalent
- Experience in complex case management and independent advanced practice
- Strong clinical skills in examination, history taking, pharmacology, and pathophysiology
- Experience with investigations and planning care pathways
- Experience working across multi-disciplinary teams and with older populations
