This opportunity offers a chance to join a multidisciplinary frailty team that provides autonomous clinical support for patients undergoing frailty health assessments, including diagnosis, treatment, and discharge within their scope of practice.
The Advanced Practitioner (AP) will deliver expert care and treatment to patients following established protocols and guidelines, with the authority to make decisions outside these frameworks when necessary based on best practice.
The role involves rotating across various locations including Stoke Mandeville Hospital, Frailty Same Day Emergency Care (FSDEC), the Frailty Hospital at Home team delivering acute care in the community, community inpatient areas at Amersham and Buckinghamshire, and the day unit at Wycombe General Hospital. The service operates 24/7, and the post holder must work flexibly to support this developing 24-hour frailty care model.
The AP will focus on health promotion and prevention, conducting comprehensive assessments to identify risk factors and early signs of illness.
Additionally, the AP will contribute to developing practice development programs for staff within the Frailty Team and the Department of Medicine for Older People.
