East Suffolk and North Essex NHS Foundation Trust is seeking an Advanced Clinical Practitioner to join our Specialist Frailty Services across North East Essex, delivering admission avoidance with a focus on preserving function.

The post supports Frailty Rapid Assessment and the Frailty Hospital@Home service, with 9.5-hour shifts over seven days and weekend work. Banding progression includes 7 and 8A as MSc credentials are completed.

Qualifications

• Autonomously assessing, planning and implementing management for undifferentiated presentations within defined scope.
• Elicit a patient history to holistically review wellbeing and needs.
• Perform clinical procedures and interpret results to form differential diagnoses and initiate treatment as required.

Responsibilities

• Assess, plan and implement comprehensive management plans.
• Elicit patient history with holistic view of wellbeing.
• Undertake physical examinations and analyse investigations.
• Perform invasive procedures and prescribing as clinically appropriate.
