Job summary

Lancashire and South Cumbria NHS Foundation Trust is seeking a Forensic Inpatient Social Care Team Manager for Guild Lodge. You will provide senior clinical leadership and line management to a team of Discharge Pathway Facilitators (Inpatient Social Workers) and work closely with a wide network of partners to ensure high-quality social care in a forensic inpatient setting.

• Qualified social worker with current Social Work England registration.
• Experience in forensic inpatient social care or related setting is preferred.

Responsibilities

• Provide senior clinical leadership and line management to the Forensic Inpatient Social Care Team.
• Oversee discharge planning and timely patient discharge by collaborating with MDTs and external agencies.
• Develop and implement service improvement initiatives within Guild Lodge.
• Lead holistic social work assessments from pre-admission and identify barriers to discharge.
• Promote patient participation and independent advocacy where applicable.
