Job description

Job Title: Floating Housing Support Practitioner

Salary: Up to 28,000

Location: Birmingham

I am currently working with a market leading charity service that offers support across Birmingham and Solihull for individuals dealing with substance misuse, including issues stemming from family-related substance misuse.

The role aims to engage individuals in various interventions through a person-centred approach, empowering them to reduce risks, build resilience, and achieve positive outcomes. This also involves motivating and encouraging individuals to access additional services or broader treatment options.

This role is working with the most vulnerable people within the community. You will support them with housing, benefits, treatment plans, signposting and interventions.

Key Responsibilities;

Provide advice, screening, brief and comprehensive assessments with individual, families or couples.

Provide brief and longer-term structured interventions.

Develop a care plan and ensure it is implemented and reviewed with the client(s) and monitor outcomes.

Provide case management and ensure integrated pathways.

Provide advice and structured support for affected others.

Provide group programmes and support groups.

Maintain written records and data.

Liaise with agencies, professionals and groups as necessary.

Participate in service promotion and information events.

Work effectively and positively as a team member.

Allocate and oversee the work of peer mentors and volunteers as required.

For your hard work and commitment, you will be paid up to 28,000, have a great holiday package which is 32 days per year plus the bank holidays, be entitled to blue light discount, pension plan, health plan and gym discounts.
