Tameside Council is seeking driven Social Workers who have completed their ASYE to join Adult Services in Hyde. The role involves managing caseloads, conducting Care Act Assessments, and developing Support Plans to help adults live well at home.

You will collaborate with health and social care colleagues to deliver coordinated, person-centred care. We offer ongoing training, supervision, and clear progression from ASYE to more senior roles.

Formación

• Qualified Social Worker with ASYE completed
• Excellent communication and relationship-building skills
• Commitment to safeguarding and adult social care

Responsabilidades

• Lead on supporting adults to live well at home within a team.
• Manage caseload including Care Act Assessments, Support Plans, and Mental Capacity Assessments.
• Signpost to internal and external services to enhance wellbeing and independence.
• Collaborate with health and social care colleagues for coordinated responses
