Central London Community Healthcare NHS Trust is seeking experienced Bank Band 6 Unplanned Care Registered Nurses to support adults in the Hertfordshire community. This role involves urgent assessment, treatment and ongoing clinical management within a community setting, working with a multidisciplinary team.

Autonomy within professional scope, with guidance, contributes to safe, person-centred care and aims to prevent unnecessary hospital admissions.

Qualificações

• Registered Nurse with current UK NMC registration.
• Practice Supervisor/Assessor or equivalent.
• Specialist Nursing Practice (District Nursing) Degree or will undertake training.
• Nurse Prescribing or will undertake training.
• Evidence of personal development.
• Knowledge of NMC code of professional conduct.
• Relevant courses in Long Term Conditions Management.
• Course of Top to Toe examination/ diagnosis Experience.

Responsabilidades

• Undertake comprehensive nursing assessments of patients presenting with urgent or unplanned healthcare needs within the community.
• Deliver safe, effective and evidence-based nursing care in accordance with Trust policies, procedures and professional standards.
• Recognise and respond appropriately to changes in patients' clinical conditions, escalating concerns promptly to the appropriate clinician or emergency services.
• Support the assessment, treatment and ongoing management of patients with acute, complex or deteriorating health needs.
• Develop, implement and review individualised care plans in collaboration with patients, carers and the multidisciplinary team.
• Administer medications and undertake clinical procedures within the limits of competence, authorisation and local policy.
