Job summary

Central London Community Healthcare NHS Trust is seeking experienced Bank Band 6 Unplanned Care Registered Nurses to support adults in the community across Hertfordshire. You will work within the Unplanned Care Service delivering urgent assessment and ongoing clinical management in a community setting, collaborating with multidisciplinary teams.

Qualifications

Registered Nurse with current UK NMC registration.
Practice Supervisor/Assessor or equivalent.
Specialist Nursing Practice (District Nursing) Degree or will undertake training.
Nurse Prescribing or will undertake training.
Evidence of personal development.
Knowledge of NMC code of professional conduct.
Relevant courses in Long Term Conditions Management.
Course of Top to Toe examination/ diagnosis Experience

Responsibilities

Undertake comprehensive nursing assessments of patients presenting with urgent or unplanned healthcare needs within the community.
Deliver safe, effective and evidence-based nursing care in accordance with Trust policies, procedures and professional standards.
Recognise and respond appropriately to changes in patients' clinical conditions, escalating concerns promptly to the appropriate clinician or emergency services.
Support the assessment, treatment and ongoing management of patients with acute, complex or deteriorating health needs.
Develop, implement and review individualised care plans in collaboration with patients, carers and the multidisciplinary team.
Administer medications and undertake clinical procedures within the limits of competence, authorisation and local policy.
