A healthcare organization in Worksop is seeking motivated Clinical Practitioners for a fixed-term contract, specifically to manage patient care during the winter months. Candidates should have experience in primary care, particularly in managing minor illnesses, and hold a Level 7 qualification along with a non-medical prescribing qualification. This position offers the opportunity to work in a supportive environment committed to high-quality patient care.
Larwood Health Partnership is a GP Practice in Worksop Nottinghamshire, and we are seeking a skilled and motivated Clinical Practitioner to join our existing team of Clinical Practitioners as part of our winter planning initiative. We are specifically looking for an experienced clinician to help support and consult our patients through the winter period (October to end of February), with a particular focus on managing seasonal and winter-related illnesses.
This is a fantastic opportunity to be part of a forward-thinking and supportive team delivering timely, high-quality care to a population of almost 35,000 patients, across four sites in the Worksop area. We are proud to be rated Outstanding by the CQC and are committed to innovation, continuous improvement, and developing new and effective ways of working to meet patient needs.
If you are passionate about patient care, especially during the high-demand winter months, and want to work in a progressive and collaborative environment, we would love to hear from you.
During the winter months, demand for healthcare services increases significantly due to seasonal illnesses. To manage this, we have previously implemented a highly successful system during the months of October to end of February in which patients are directed to attend the surgery and "Sit and Wait" to be seen by one of our Clinical Practitioners.
You will be joining a dynamic team of Clinical Practitioners from both nursing and paramedic backgrounds, who manage a wide range of presentations, including minor illnesses and long-term conditions. You will hold a prescribing qualification and a Level 7 qualification, such as APACS or a Minor Illness/Injury course. Paediatric experience would be desirable but is not essential for this role.
We are a SystmOne Practice and use SystmConnect to facilitate a total triage model for all appointments. In this role, you will assess, diagnose, treat, and manage minor illnesses presenting on the day. In the main these are patients presenting with chest, throat or ear infections. These consultations will be face-to-face.
We have a multidisciplinary team made up of:
Conduct thorough clinical assessments, order appropriate investigations, diagnose, and formulate treatment plans, including onward referrals where appropriate for patients presenting with minor illness symptoms, and those with known long-term conditions.
Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition as appropriate.
Provide health promotion and education to patients around self-care and appropriate use of services.
Recognise and act on red flags and signs of serious illness, referring appropriately when needed.
Work collaboratively with GPs, practice nurses, pharmacists, and wider primary care teams.
Maintain accurate and timely clinical documentation in line with practice and legal requirements.
Contribute to the development of the minor illness service, supporting innovation and service improvement.
Participate in clinical audits, reviews, and quality improvement activities.
Be aware of health inequality and government agendas such as Core20PLUS5 with the aim of reducing health disparities between groups of people and improving access to healthcare.
Adhere to safeguarding policies and procedures to ensure patient safety.
