Job description

You will work across TeesdalePCN which included 6 GP practices, to support the diagnosis and treatment of patients with musculoskeletalconditions. You will act as a first point of contact for patients with newmusculoskeletal conditions, offering evidence based treatment to help managethese conditions. You will offer expert advice to other members of the practiceteam, including GPs and nurses, to support high quality patient care. You willhave a minimum of 5 years post graduate MSK experience in an NHS or MSK role,and will be able to work independently without direct supervision in this role.

Main duties of the job

Be the firstpoint of contact for expert clinical assessment and diagnosis of patientspresenting with MSK conditions in GP practice. This will include patients whopresent with a range of multiple needs and underlying pathologies/other primaryconditions

Use ofadvanced clinical assessment tools andapplication of expert treatment modalities/techniques will be required

The linkbetween primary, community and acute services, This will include provision of triage to other servicesand/or directly providing management and advice

Demonstrateeffective communication between primary care services/GP practices and otherrelevant care providers. Educate others on the role of the FCP

Workindependently, without daily supervision to assess, diagnose, triage, andmanage patients, taking responsibility for prioritising and managing a caseloadof the PCNs Registered Patients

Receivepatients who self-refer or from a clinical professional within the PCN, andwhere required refer to other health professionals within the PCN

Work as partof a multi-disciplinary team in a patient facing role, using their expertknowledge of MSK physiotherapy

Developintegrated and tailored care programmes in partnership with patients, providinga range of first line treatment options including self-management, referral torehabilitation focussed services and social prescribing

Job responsibilities

Use advancedclinical practice skills and clinical reasoning to provide comprehensivediagnostic and treatment support patients who present with complexmusculoskeletal conditions and/or multiple pathologies and/or mental healthand/or pain management needs

Request and progress referrals andinvestigations to facilitate the diagnosis and choice of treatment regime,considering the limitations of these investigations, interpret and act onresults and feedback to aid patients diagnoses and management plans

Carry owncaseload as an autonomous practitioner, providing directclinical/physiotherapeutic care for patients with a range of complexmusculo-skeletal conditions and who may have significant other primary orsecondary conditions/multi-pathologies

Be able todecide when appropriate to use advanced clinical practice clinical skills thatmay include:

Joint/softtissue injection therapy

Useprofessional judgement and advanced clinical reasoning skills to make decisionsabout safe and effective patient care in unpredictable situations, includingwhen there is incomplete/contradictory information

Demonstrateunderstanding of the impact of physiotherapeutic interventions on existingconditions and treatment programmes

Ensurephysiotherapy interventions are integrated and supportive of the wholetreatment aims promoting a holistic approach to condition management

Be able toclinically justify referrals onto appropriate MSK pathways within the communityor wider healthcare services, including referral to the appropriate stage ofthe pathway

Justifyclinical referrals to other specialist services. This will include referral toacute and specialist services such as surgical orthopaedics, rheumatology andneurology, other AHP services, nursing services and to the primary care medicalteams. This requires advanced knowledge of the role of other specialist healthprofessionals

Recognise Redflags, serious pathology and potential underlying non- MSK related disease,that may present as MSK symptoms and refer appropriately.

When needed,accelerate the patients referral to other health professionals/ services

Work inpartnership with the patient at all times to attain maximum participation intreatment programmes. This will include working with patients from diversesocial background and cultures and understanding how this will affect treatmentproposals and models

Use effectiveshared decision making with a range of first line management options(appropriate for the persons level of activation)

Assess levelsof Patient Activation to confirm levels of knowledge, skills and confidence toself-manage

Agreeappropriate support for self-management through referral to rehabilitationfocussed services and social prescribing provision

Provideexpert advice and act as source of expertise in the management of musculoskeletalconditions

Provideadvice, instruction and teaching on aspects of management of a condition topatients, relatives, carers and other health professionals

Beaccountable for decisions and actions via Health and Care Professions Council(HCPC) registration, supported by a professional culture of peernetworking/review and engagement in evidence-based practice

Recognise and work within own competence andprofessional code of conduct as regulated by the HCPC

Deliver care according to NSF, NICE guidelinesand evidence-based care

Assess effectiveness of care delivery throughself and peer review.

Partake in the audit cycle as a means ofevaluating the quality of the FCP service

Actively participate in the DDHF significantevent procedures.

Understandand apply legal issues that support the identification of vulnerable and abusedchildren and adults, and be aware of statutory child/vulnerable patients healthprocedures and local guidance.

Prioritisework load and work efficiently in the FCP service, ensuring delivery in linewith service expectations

Workcollaboratively with the primary care team, including working across the PCNand other care providers to improve service delivery and meet the identifiedneeds of the local patient population.

Work with thePCN team to support preventative strategies for local populations, patientgroups and individuals

Participatein peer networking and support underpinned by active engagement in peer reviewand embracing evidence-based practice

Demonstrate adetailed knowledge of wider health, healthcare and social care agencies relatedto MSK conditions and physiotherapy.

Takeresponsibility for identifying and reporting any risk/ clinical governanceissues in the FCP service

Represent thePCNs FCP service at internal primary care meetings

Be aware of and able to demonstrate how the FCProle contributes to the achievement of Key performance Indicators (KPIs) forMSK primary care services

Provide andreceive clinical supervision and be an active provider of mentoring andcoaching of specialist clinicians from a range of disciplines.

Contribute totraining and supervision, some of which maybe to postgraduate level, forspecialist primary care professionals, including GPs in order for them todevelop specialist MSK patient referral and management skills

Maintain aprofessional portfolio of advanced clinical practice knowledge and skillsthrough participation in internal and external development opportunities

  • Experience in applying the physiotherapy process to include MSK assessment, interpretation, individual care planning, motivation, monitoring and evaluation of highly specialized MSK treatment

  • Ability to integrate latest evidence based practice into services
