A healthcare organization in Warrington is seeking a qualified physiotherapist to manage patient care within a Primary Care Network. Ideal candidates will have over 3 years of MSK experience and will work flexible hours, including evenings and weekends. Responsibilities include leading clinical pathways and supporting the integration of multidisciplinary teams.
The postholder will undertake this role for the 7 GPPractices of Central & West Warrington Primary Care Network (CWW PCN), asper rota scheduling to manage the caseloads of allocated Practice(s) anddelivering the defined requirements to meet the Primary Care Network DES and asper core Practice requirements.
This post will be managed by a Lead Practice with sessionsand clinical work taking place across the Practice(s) weekly, as per agreedscheduled rotas.

It is essential that candidates have 3 years +consolidated Musculoskeletal (MSK) experience. Your application will not beconsidered unless you can meet this requirement
This position will include working evening hours (upto 8pm) on a Thursday and regular Saturdays (9am-5pm)
Hours per week between 15-22.5
**Salary is Whole Time Equivalent (WTE) dependent uponexperience**
Work across the multi-disciplinary team to create andevaluate effective and streamlined clinical pathways and services.
Provide leadership and support on MSK clinical and servicedevelopment across the PCN, alongside learning opportunities for the wholemultidisciplinary team within primary care.
Develop relationships and a collaborative working approachacross the PCN, supporting the integration of pathways in primary care.
Provide clinical expertise, acting as first-contactphysiotherapist and making decisions about the best course of action forpatients care (including in relation to undifferentiated conditions). This willinvolve seeing patients, without prior contact with their GP, in order toestablish a rapid and accurate diagnosis and management plan.

They will be part of our established FCP team. Supportis given by the FCP team, the employing Practice
Manager,the PCN Clinical Director and the PCN Manager.
Take professional responsibility as a first-contactphysiotherapist, with high-level decision-making and clinical-reasoning skillsto assess, diagnose and triage of patients.
Work independently, without day to day supervision, toassess, diagnose, triage, and manage patients, taking responsibility forprioritising and managing a caseload of the PCNs Registered Patients.
Manage a complex caseload (including patients with long termconditions, co-morbidities and multi-factorial needs).
Receive patients who self-refer (where systems permit) orfrom a clinical professional within the PCN, and where required refer to otherhealth professionals within the PCN
Leads, manages and contributes to service delivery,including attending Practice meetings and other meetings (e.g. PPG/ICS) asappropriate to report on workstreams.
Encourage collaborative working across the wider healtheconomy and be a key contributor to supporting the development of physiotherapyclinical services across the PCN.
Liaise with secondary and community care services, andsecondary and community MSK services where required, using local social andcommunity interventions as required to support the management of patientswithin the PCN; and support regional and national research and audit programmesto evaluate and improve the effectiveness of the FCP programme. This willinclude communicating outcomes and integrating findings into own and widerservice practice and pathway development.
Work as part of a multi-disciplinary team in apatient facing role, using their expert knowledge of movement and functionissues, to create stronger links for widerservices through clinical leadership, teaching andevaluation.
Develop integrated and tailored care programmes inpartnership with patients, providing a range of first line treatment optionsincluding self-management, referral to rehabilitation focussed services andsocial prescribing.
Make use of their full scope of practice, developing skillsrelating to independent prescribing, injection therapy and investigation tomake professional judgements and decisions in unpredictable situations,including when provided with incomplete or contradictory information. They willtake responsibility for making and justifying these decisions.
Manage complex interactions, including working with patientswith psychosocial and mental health needs, referring onwards as required andincluding social prescribing when appropriate.
Communicate effectively with patients, and their carerswhere applicable, complex and sensitive information regarding diagnoses,pathology, prognosis and treatment choices supporting personalised care.
Implement all aspects of effective clinical governance forown practice, including undertaking regular audit and evaluation, supervisionand training.
Request and progress investigations (such as x-rays andblood tests) and referrals to facilitate the diagnosis and choice of treatmentregime including, considering the limitations of these investigations,interpret and act on results and feedback to aid patients diagnoses andmanagement plans; and Develop integrated and tailored care programmes inpartnership with patients through:
Effective shared decision-making with a range of first linemanagement options (appropriate for a patients level of activation).
Assessing levels of Patient Activation tosupport a patients own level of knowledge, skills and confidence to self-managetheir conditions, ensuring they are able to evaluate and improve theeffectiveness of self-management interventions, particularly for those at lowlevels of activation.
Agreeing with patients appropriate support forself-management through referral to rehabilitation focussed services and widersocial prescribing as appropriate; and designing and implementing plans thatfacilitate behavioural change, optimise patients physical activity andmobility, support fulfilment of personal goals and independence, and reduce theneed for pharmacological interventions.
Help plan, develop and support the introduction of newworking processes within the Practice to optimise the service.
Accountable for decisions and actions via HCPC registration,supported by a professional culture of peer networking/review and engagement inevidence-based practice.
Streamline pathway of care by providing a responsive serviceso that patients receive timely access to care.
Ensure care is proactive, preventative in focus andpopulation based, with an emphasis on early intervention.
Provide care which is tailored to the individual needs. Thiswould include appraising the impact of individuals clinical status on theirgeneral health, well-being, employment status (including in relation tofunction, physical activity, mobility and independence).
Support patients to set their own goals and be confident intheir approach to self-management.
Communicate effectively and appropriately with patients andcarers complex and sensitive information regarding diagnosis, pathology andprognosis.
Use a range of clinical skills which may include:
o Non-medical independent prescribing
o Joint aspirations
Provide learning opportunities for the whole multiprofessional team within primary care.
Work with Multi-Disciplinary Team (MDT) to develop moreeffective and streamlined clinical pathways and services.
Exercise professional judgement, making, justifying andtaking responsibility for decisions in unpredictable situations, including inthe context of incomplete/contradictory information.
Manage interactions in complex situations, including withindividuals with particular psychosocial and mental health needs and withcolleagues across the primary care team, sectors and settings.
Undertake any other duties commensurate with the postholdersgrade as agreed with the postholders line manage.
