NHS seeks a dedicated First Contact Physiotherapist to provide patient-centered care in Hounslow. The ideal candidate will assess, diagnose, and manage patients directly, collaborating with a multidisciplinary team to optimize health outcomes and integrate patient-self management strategies.
We arecurrently looking for an enthusiastic, highly motivated, proactive and forwardthinking First Contact Physiotherapist, committed to providing highstandards of holistic clinical care and professional conduct to our patientspopulation.
The right candidate needs to beable to work effectively within a collaborative and integrated MDT environmentand will contribute to the patients care and management with a view to promoting care planning and proactive patientmanagement initiatives.
1. Provide clinical expertise, acting as first-contact physiotherapist and making decisionsabout the best course of action for patients' care (including in relation to undifferentiatedconditions). This will involve seeing patients, without prior contact with their GP, in orderto establish a rapid and accurate diagnosis and management plan.
2. Progress and request investigations to facilitate diagnosis and choice of treatmentregime, understanding the information limitations derived from these and the relativesensitivity and specificity of particular tests diagnostic services such as x-rays and bloodtest, and interpret and act on results to aid diagnosis and the management plans ofpatients.
3. Deliver programmes of supported patient self-management, in ways that facilitatebehavioural change, optimise individuals' physical activity, mobility, fulfilment ofpersonalgoals and independence, and that minimise the need for pharmacologicalinterventions

1. Take professional responsibility as a first-contact physiotherapist, with high-leveldecision-making and clinical-reasoning skills to assess, diagnose and triage of patients
2. Manage a complex caseload (including patients with long-term conditions, co-morbidities and multi-factorial needs)
3. Lead, manage and contribute to service delivery
4. Be accountable for decisions and actions via HCPC registration, supported by aprofessional culture of peer networking/review and engagement in evidence-basedpractice
5. Streamline pathway of care by providing a responsive service so that patients receivetimely access to care
6. Ensure care is proactive, preventive in focus and population based, with an emphasison early intervention
7. Provide care which is tailored to the individual needs. This will include, appraisingthe impact of individual's clinical status on their general health, well-being, employmentstatus (including in relation to function, physical activity, mobility and independence
8. Support patients to set their own goals and be confident in their approach to self-management
9. Communicate effectively and appropriately with patients and carers complex andsensitive information regarding diagnosis, pathology and prognosis.
10.Use a range of clinical skills which may includea. non-medical independent prescribingb. joint / soft tissue injectionsc. joint aspirations
11.Provide learning opportunities for the whole multi professional team within primarycare.
12.Work with MDT to develop more effective and streamlined clinical pathways andservices
13.Exercise professional judgement, making, justifying and taking responsibility fordecisions in unpredictable situations, including in the context ofincomplete/contradictory information
14.Manage interactions in complex situations, including with individuals with particularpsychosocial and mental health needs and with colleagues across the primary careteamand settings
