Job summary

A community support organization in Wrexham is seeking a passionate Fieldcare Supervisor to manage a team and enhance the quality of life for individuals receiving support. The role includes developing care plans and leading a dynamic team. Applicants should have a Level 3 qualification in Health & Social Care and relevant management experience. Qualifications

  • Previous experience in a care management role, ideally within CIW.
  • Experience working with individuals with autism, acquired brain injury, complex mental health needs, traumatic backgrounds, and challenging behaviours.

Responsibilities

  • Manage a team of colleagues to help people gain more independence.
  • Develop and implement person-centred support plans.
  • Induct new colleagues and complete supervisions/appraisals.
  • Attend multi-disciplinary meetings and assist in service management.
  • Regularly visit services and spend time with supported individuals.
  • Participate in the on-call rota.

Fieldcare Supervisors with a passion for enhancing the quality of life of the people we support are at the heart of our community service teams. We’re currently searching for a Field Support Supervisors to join us in leading our supported living service(s) in Wrexham. This service is a welcoming and friendly environment with a dynamic team that supports a fantastic group of people!

Responsibilities
  • Manage a team of colleagues to help people gain more independence in their daily lives.
  • Develop and implement person-centred support plans and conduct care needs assessments.
  • Induct new colleagues, complete supervisions/appraisals, and carry out medication competencies and audits.
  • Attend multi-disciplinary meetings, complete reports, and assist the Branch Manager in day-to-day service management.
  • Regularly visit services and spend time with the people we support.
  • Participate in the on-call rota (subject to service needs).
Requirements
  • Previous experience in a care management role, ideally within CIW.
  • Level 3 in Health & Social Care.
  • Experience working with individuals with autism, acquired brain injury, complex mental health needs, traumatic backgrounds, and challenging behaviours.
