Work well with customers
• Prior to each service commencing, carry out an assessment with customers and/or their chosen representatives to understand their needs and preferences, along with a risk assessment.
• Create and agree on a written, individually tailored plan of care and support that respects the customer’s wishes and promotes their dignity and privacy. Keep care and support plans up to date.
• Provide customers and, where appropriate, their representatives with information about the service, ensuring clarity on expectations and how to raise any concerns.
• Keep all information about customers and their families secure and confidential.
Work well with staff
• Provide care assistants with appropriate information and guidance to effectively and safely deliver care as per the care and support plan.
• Collaborate with the Bluebird Care coordinator to match care assistants to customers and manage the workflow. Attend the customer's first call to introduce the care assistant. Report changes to care schedules promptly to the coordinator, customer, and care assistant.
• Distribute, collect, and monitor MAR charts.
• Maintain up-to-date staff files and records.
• Offer advice, inductions, training, supervision, and support to the team of supervisors and/or care assistants.
• Participate in the out-of-hours emergency on-call rota as required, handling emergency on-call issues, such as covering calls when care assistants are sick or absent. Record emergency calls and communicate information to relevant parties.
• Arrange and manage staff changeovers and briefings for live-in care assignments in the absence of the care manager.
Quality improvement
• Conduct investigations related to service quality and implement improvement actions.
• Monitor care assistants' performance and carry out quality reviews at customers' homes to ensure service delivery aligns with company procedures.
• Adhere to Bluebird Care policies and procedures at all times, ensuring accidents and incidents are recorded, reported, and addressed.
• Ensure care and support are provided in compliance with regulatory requirements and relevant legislation.
• Collaborate effectively with customers, their families, and other social and health care professionals to deliver high-quality home care services.
