Job summary

Radis Community Care is seeking a Field Supervisor to join the team in Tonypandy. You will work in the community, conducting reviews, spot checks, and carer assessments, while ensuring clients stay in the home they love. You will coordinate with NHS teams and other health professionals to meet client needs.

The role involves on-call duties (1 in 4 weeks) and is offered with both part-time supervision duties and full-time care duties. A valid driving licence and vehicle access are required.

Qualifications

• 1 year experience in Health & Social Care.
• Enhanced DBS check required.
• Driving licence with access to a vehicle is required.

Responsibilities

• Conduct monitoring tasks such as reviews, spot checks, and carer assessments.
• Complete Risk Assessments, MAR charts and support plans.
• Be the face of Radis in the community and liaise with NHS, Social services, GP and other health professionals.
• Participate in On call (1 in 4 weeks).

About the role

Part time supervisor duties

Full time available with care duties

As a Field Supervisor you will work out in the field to complete monitoring tasks such as reviews, spot checks, carer assessments as well as completing Risk Assessments, MAR charts and support plans.

You will be the face of Radis working in the community ensuring people stay in the home they love. You will also be working with the NHS, Social services, GP's and other health professionals to ensure all of our clients needs are met.

You would also take part in On call which would be 1 in 4 weeks.

Salary

All Supervisor Hours are to be paid at an enhanced hourly rate, any care completed would be paid at our care rate and you would also receive paid travelling time in between care visits as well as paid mileage. When completing on call you would also receive an On call Payment.

Skills and Qualifications required for the role

• 1 years experience in Health & Social Care
• The successful candidate will have an NVQ Level 2 in Health and Social Care with a view to completing Level 3.
• You will have the ability to set and maintain high standards of quality and professionalism in all areas of work.
• You will have good written and verbal communication skills with the ability to engage with staff, service users and outside agencies in a professional manner.
• This role requires you to undergo an enhanced DBS check.
• A driving licence with access to a vehicle will also be required for the role.
