Job Title: Field Care Manager
Location: North West
Salary: £27,000 Per Annum
Reports to: Senior Clinical Lead for Region
Working Hours: 40 hours per week (with some flexibility over weekends/evenings depending on client needs)

About the Opportunity:

We are looking for a passionate and dedicated Field Care Manager to oversee the delivery of a high standard of home care services. Your responsibilities will include conducting client assessments and managing care plans, ensuring our services meet compliance with CQC regulations. The role involves regular and frequent travel to clients’ homes and could include some evenings and weekends.

Key Responsibilities:

  • Care Planning & Assessment: Develop and review personalised care plans.
  • Liaise with the operational team regarding performance issues and training needs.
  • Compliance & Quality Assurance: Ensure services meet CQC standards, conduct audits, and maintain accurate records.
  • Build relationships with clients and their families, address concerns, and update their care needs.
  • Collaborate with healthcare professionals to ensure holistic care.

Requirements:

  • NVQ Level 3 or 4 in Health & Social Care (or equivalent).
  • Experience in a supervisory/management role within health and social care.
  • A passion for delivering high-quality care, exceptional communication, and organisational skills.
  • Full UK driving license and ability to travel using own car.
  • This role requires travel around the North West including North Wales, Liverpool, Manchester, Stoke, and surrounding areas.

  • A competitive salary of £27,000 depending on skills and experience.
  • Remote/field-based role.
  • Working hours – 9 am - 5:30 pm Monday to Friday with some flexibility over weekends/evenings.
