Surrey Downs Health and Care in Leatherhead is seeking a Band 5 Nurse to join the community nursing team serving the Leatherhead area. You will assess, plan, implement and evaluate holistic care for patients in their own homes or local clinics, working as part of an established district nursing team.

Qualifications

• Post-registration professional development evidenced.
• Understand NHS issues and clinical governance.
• Some community nursing experience.

Responsabilités

• Work as part of the team with lead responsibilities as negotiated with District Nurse.
• Assess care needs, implement and evaluate holistic programmes of care.
• Involve, support, inform and educate family/carers.
• Maintain clear, signed and contemporaneous records per SDHC procedures.
• Be responsible for timely patient care and data entry.
