Libertatem Healthcare is seeking to recruit experienced Female Support Workers due to our clients' needs. Our client is in his 60s, lives in his property, and has a brain injury. He suffers from memory impairment and dyslexia. He is a smoker and requires support in the community, attending appointments, and domestic duties.

Hours:

3 shifts a week

Day shift

10:00-15:00 or 11:00-16:00

Main Tasks and Duties:

  1. Supporting independence and continued rehabilitation.
  2. Providing personalised support in a sensitive and confidential manner, according to the support plan.
  3. Understanding and following care plans, seeking clarification from healthcare professionals as necessary.
  4. Participating in monitoring and reviewing Service Users’ care plans.
  5. Working professionally with all members of the multidisciplinary team.
  6. Promoting self-care and self-management to maximise independence.
  7. Assisting with domestic tasks: laundry, ironing, cleaning.
  8. Preparing food and drinks, kitchen hygiene, shopping, and menu planning.
  9. Helping Service Users attend appointments, go shopping, and collect pensions.
  10. Providing emotional support to boost confidence and motivation.
  11. Assisting with moving and handling equipment for safety and independence.
  12. Monitoring skin integrity and reporting changes.
  13. Administering medication as per care plan and company policies.
  14. Supporting rehabilitation activities as directed.
  15. Monitoring health and well-being, responding to changes indicating potential issues.
  16. Delivering support respecting cultural and personal differences.
  17. Documenting progress and changes in daily records.

An enhanced DBS check and full training will be provided, supported by our clinical lead nurse.
