Job summary

The Children’s Society is seeking a qualified Family WrapAround Practitioner to join our Warwickshire team. Based at Rugby or Stratford, you will work with families to improve access to support through a multidisciplinary approach and build strong relationships with partner services.

The role involves developing personalised wraparound care plans, coordinating with health, education and social care, and empowering families to manage emotional and mental health needs.

Qualifications

• Experience in family support with complex social, emotional and SEMH needs.
• Proven ability to collaborate with health, education and social care teams.
• Experience engaging hard-to-reach families in community settings.
• Experience advocating for vulnerable children and families, coordinating holistic care plans.

Responsibilities

• Develop and maintain wraparound care plans for families.
• Build relationships with professionals across organisations to enhance services.
• Coordinate and broker access to emotional and mental health supports.
