Salary: £25,000 per annum (pro-rata for part-time)

Contract/Hours: Fixed Term Contract until 31 March 2026, Part-time- 30 hours per week

About the Service

Reflect offers support to women whose children are in foster care or have been adopted.

It aims to help them as their circumstances change and supports them in positive future choices. Each women's journey is unique and with support and understanding they can successfully renegotiate their role at different stages of that journey.

The service offers support by:

  • Building emotional resilience
  • Improving confidence and self-esteem
  • Developing social networks
  • Support to attend LAC reviews
  • Support to attend health appointments
  • Keeping themselves safe
  • Accessing community facilities and resources
  • Supporting contact visits
  • Support to attend court proceedings and legal meetings.

About the role

As a Family Support Practitioner, you'll provide a wide range of services across Carmarthenshire, which enables positive outcomes to be achieved.

This will be achieved in line with organisational policies, procedures and regulatory requirements.

How you'll create brighter futures

  • Deliver a complex and specialist interventions to women whose children have been fostered or adopted on a 1:1 basis.
  • Provide care planning management, including undertaking assessing, reviewing and managing risks (Carmarthenshire Region)
  • Attending and participating at relevant multi agency meetings, providing a lead role ensuring reflective and analytical practice review
  • Take responsibility for day-to-day resource planning to ensure high quality services are delivered.
  • Establish and developing effective relationships and channels of communication with children, young people, families, colleagues, community organisations, partner agencies and other external agencies

Let's talk about you

  • Willing to travel across region (Carmarthenshire)
  • Experience of chairing, supporting and contributing to service planning meetings.
  • Experience of the development and practice of community working, including influencing involvement
  • Ability to lead and develop service implementation plan
  • Working in collaboration to achieve changewithin your immediate team and wider multi agency partnerships
  • Experience of collating and producing reports, capturing and progressing the relevant information for a desired outcome
  • UK driving license with access to transport, for the purposes of transportation of service users to appointments and meetings
