Making a Difference as a Family Hub Outreach Practitioner

This role supports the implementation of the Family Hub model in St Helens. You will work alongside the Family Hub Manager as part of the team, ensuring high standards of care, education, and support for children, young people, and families, in accordance with current legislation.

Your responsibilities include planning and organizing for key children within specific age groups in the Family Hubs and Children’s Centres, conducting ongoing assessments, training, and monitoring of emotional wellbeing, progress, and development, and reporting outcomes to the Family Hub Manager.

The Family Hub Outreach Practitioner will be the first point of contact for children and families, facilitating smooth transitions to relevant services. Working closely with managers and partners, you will support the development of Family Hubs and the Early Help offer, promote services to diverse audiences, and identify opportunities for service development to meet community needs.
