Role Overview:

  • Enable families to receive timely interventions across the 0-19 age range, planning, delivering, and reviewing support as per the Family Help Framework.
  • Support the setup and delivery of evidence-based programmes to improve family outcomes.
  • Undertake holistic assessments of family situations and plan, deliver, and review support programs, prioritizing vulnerable children and hard-to-reach families.
  • Maintain accurate records in the case management system for audit, information sharing, and reporting.
  • Work collaboratively with children’s services and other agencies to deliver targeted, coordinated programmes.
  • Identify and support families facing difficulties, sharing insights with line managers to monitor and evaluate services.
  • Ensure active involvement of parents, carers, and children’s voices in all interactions.
  • Comply with statutory, departmental, and safeguarding policies to ensure children’s safety.
  • Engage young people across services to re-engage them with provision and support.
