£51,883 - £58,544 pa pro rata inclusive of HCAS
We are looking to recruit a Falls Specialist Physiotherapist to join the Adult and Urgent Community Response (AUCR) Directorate at Bromley Healthcare.
This role is suited to an experienced physiotherapist with specialist expertise in falls prevention and the rehabilitation of older adults. The postholder will play a key role in delivering high-quality, community-based care as part of the Falls and Fracture Prevention Service, including triaging referrals, leading clinical assessments, coordinating care, and supporting the delivery of individual and group-based falls prevention interventions.
The post involves close working with multidisciplinary teams across the directorate, including nursing, occupational therapy, reablement, and social care, to ensure coordinated and person-centred support for individuals at risk of falls and living with frailty.
Main duties of the jobTriage and clinical management of referrals to the Falls and Fracture Prevention Service.
Undertake specialist physiotherapy assessments and interventions for adults at risk of falls, including those living with frailty and complex health needs.
Deliver personalised rehabilitation programmes in a variety of settings, including patient homes, clinics, care homes, and group therapy environments.
Facilitate and lead structured falls prevention groups, such as strength and balance programmes.
Provide advice, education, and support to patients, families, and carers to reduce falls risk and promote independence.
Work collaboratively with multidisciplinary teams across community services, primary care, and the voluntary sector.
Support the development of falls care pathways and contribute to service improvement initiatives.
Supervise and support junior staff, students, and rehabilitation assistants within the service.

Compensation is contingent upon NHS experience and current banding/pay point.
