Job overview

Due to retirement, an opportunity has arisen for a motivated, adaptable healthcare or therapy assistant to join the Falls Prevention Team, working across Bradford and Airedale.

We seek enthusiastic, organized, and flexible individuals with excellent communication skills to work independently under supervision, supporting falls prevention and anticipatory care for older adults in their homes.

The team delivers community-based, integrated healthcare, combining preventative and recovery interventions. We are implementing new ways of working to enhance care.

Main duties of the job
  1. Work as part of the Falls Prevention Team across the organization
  2. Contribute to service development collaboratively
  3. Provide high-standard, evidence-based, and effective care
  4. Support, advise, and educate colleagues, patients, and carers on falls prevention
  5. Use holistic assessment processes for patients at risk of falls
  6. Act as a positive role model
  7. Develop personal knowledge and skills to adapt to service changes
  8. Support multidisciplinary team in planning, implementing, and reviewing care programs related to falls prevention and bone health
