Job summary

Sefton Metropolitan Borough Council is seeking an Experienced Social Worker in the Adult Social Care team in Sefton. You will manage a complex caseload of adults with care and support needs, conducting assessments, reviews and support planning in line with relevant legislation.

Working with individuals, families, carers, health colleagues and partner agencies, you will develop person-centred support plans that promote independence, wellbeing and positive outcomes.

Qualifications

• Experience delivering adult social care assessments and care planning.
• Knowledge of Care Act 2014 and Mental Capacity Act 2007.
• Ability to work with individuals, families and partner agencies.
• Strong record keeping and reporting skills.

Responsibilities

• Manage a complex caseload of adults with care and support needs, conducting assessments, reviews and support planning in line with Care Act 2014 and MCA 2007.
• Work with individuals, families, carers, health colleagues and partner agencies to develop personalised support plans that promote independence and wellbeing.
• Undertake assessing under The Care Act through the 3 Conversations Model, complete safeguarding enquiries, assess risk and coordinate care packages.
• Make sound professional decisions, maintain accurate records and reports, and contribute to a collaborative team delivering high-quality social work practice.

As an Adult Social Worker within the South Community Team, you will manage a complex caseload of adults with care and support needs, completing assessments, reviews, and support planning in line with relevant legislation, including the Care Act 2014, Mental Capacity Act 2007.

You will work closely with individuals, families, carers, health colleagues, and partner agencies to develop personalised support plans that promote independence, wellbeing, and positive outcomes. The role includes undertaking assessing under The Care Act through the 3 Conversations Model, completing safeguarding enquiries, assessing risk, coordinating care packages, and ensuring that support remains effective through regular review.

You'll be expected to make sound professional decisions, maintain accurate records and reports, and contribute to a collaborative team environment focused on delivering high-quality social work practice which is underpinned by legislation, theory and policy.
