You will be joining KMPT at an exciting time. The work around the implementation of the Community Health Framework, which will enable Social Work to develop a strong profile within secondary care mental health service to improve patient experience and complement our existing MDT.
As a Social Worker, you will provide a high standard of support and care delivery with regard to social care needs to those service users whose ability to cope with the tasks of living are threatened or impaired by mental health issues, emotional, psychological and social problems.
You will be working in the West Kent Home Treatment Team, located at Priority House, Maidstone. The Team covers the West part of Kent and this includes Maidstone, Tunbridge Wells and Edenbridge.
The post holder must hold a full, valid, UK driving licence and have access to a car/vehicle to use for business purposes (unless you have a disability as defined by the Equality Act 2010).
You will work closely with other agencies and individuals to support service users with a clear focus on recovery, social inclusion and social justice. The role will entail assessment, support planning and the opportunity to provide 1-1 strength-based therapeutic interventions.
You will act as Lead Health Care Professional, (where applicable) with individuals who have serious and enduring mental health difficulties.
