Job Title: Experienced Social Worker (AWLD Transition Service)
Location: Lewisham
Rate: Up to £35 per hour

An excellent opportunity has become available for an Experienced Social Worker to join the AWLD Transition Service team at Lewisham Borough Council. This team is dedicated to supporting young adults aged 18-25 with learning disabilities, autism, and physical disabilities as they transition to adulthood.

Responsibilities include:

  • Assessing and supporting vulnerable young adults with learning disabilities, autism, and physical disabilities during their transition to adulthood.
  • Using strength-based assessment techniques and providing tailored guidance to meet social care needs.
  • Ensuring compliance with relevant legal frameworks, such as completing Community DoLS assessments.
  • Collaborating with other professionals to achieve positive outcomes for service users.

Qualifications and Experience:

Minimum of 2 years post-qualification experience as a Social Worker, with experience working with young people with learning disabilities, autism, and physical disabilities. Must have Social Work England registration and be eligible to work in the UK.
