An Integrated Care System in Liverpool is seeking an experienced Practice Nurse to enhance patient care within their dynamic team. This full-time position involves delivering clinical care and chronic disease management while collaborating with healthcare professionals. iGPc Primary Care Network is looking for a PracticeNurse to join our forward-thinking Primary Care Network team (Full time)supporting a new housebound service.
This role will be focused onproviding proactive reviews and clinical care to our patient population, byproviding both chronic disease reviews and proactive holistic assessments toour housebound population as part of a clinical team, as well as more traditionalPractice-based work on cervical screening and chronic disease reviews such asAsthma, Hypertension and health checks.
The successful candidate willjoin a team of Nurses and work alongside GPs, Pharmacists, ANPs, Paramedics aswell as a non-clinical team including Social prescriber Link workers and CareCo-ordinators.

Hours of work are negotiable andwill include Saturdays.
The postholder, an experienced Practice Nurse acting within their professionalboundaries, will provide care for the presenting patient from initial historytaking, clinical assessment, diagnosis, treatment and evaluation of their care.They will demonstrate safe, clinical decision-making and expert care forpatients within the patients home or GP practice. They will workcollaboratively with the multi-disciplinary general practice team to meet theneeds of patients, supporting the delivery of policy and procedures, andproviding nurse leadership as required. The collaboration of ourconstituent GP Practices is designed to address the healthcare needs of thelocal community by making full use of the resources and hands on experienceof our Practices, as well as other healthcare providers and partners.

JOB SUMMARY
The post holder is an experienced Practice Nurse, who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care. They will demonstrate safe, clinical decision-making and expert care for patients within the patient's home or GP practice. They will work collaboratively with the multi-disciplinary general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required. In order to work at this level, NMC requirements for Practice Nursing must be evidenced and maintained
1. PRIMARY DUTIES & AREAS OF RESPONSIBILITY
A. CLINICAL ROLE:
Acting within their professional boundaries and competencies, the post-holder will:
Participate in identification of community health needs and develop patient/family-centred strategies to address them
Help develop and set up new patient services and participate in initiatives to improve existing patient services
B. TEACHING AND MENTORING ROLE:
The post-holder will:
Promote a learning environment for patients, nurses and other health professionals.
Contribute to the planning and implementation of the teaching for existing staff, such as medical students, NP students, GP registrars, Health Care Assistants, within the Practice.
C. PROFESSIONAL ROLE:
The post-holder will:
Promote evidence-based practice through the use of the latest research-based guidelines and the development of practice-based research.
Monitor the effectiveness of their own clinical practice through the quality assurance strategies such as the use of audit and peer review
Maintain their professional registration
Participate in continuing professional development opportunities to ensure that up-to-date evidence-based knowledge and competence in all aspects of the role is maintained
Work within the latest NMC Code of Professional Conduct
Record accurate consultation data in patients records in accordance with the latest NMC guidance and other pertinent standards
Keep up to date with pertinent health-related policy and work with the practice team to consider the impact and strategies for implementation
Work collaboratively with colleagues within and external to the practice.
Demonstrate leadership
Pro-actively promote the role of the Practice Nurse within the PCN and its member practices , and externally to key stakeholders and agencies
2. HEALTH AND SAFETY/RISK MANAGEMENT
The post-holder must comply at all times with the Practices Health and Safety policies, in particular by following agreed safe working procedures and reporting incidents using the organisations Incident Reporting System.
The post-holder will comply with the Data Protection Act (1984) and the Access to Health Records Act (1990).

The post-holder will establish and maintain effective communication pathways with the following :
GPs and other clinicians of the Practices and PCN including Pharmacy and Advanced Practitioner colleagues
Team Leaders of the PCN
Practice Nurses within the Practices
Practice Managers
Admin/Reception teams
PCN Management team
5. SPECIAL WORKING CONDITIONS
The post-holder is required to travel independently during home visits (where applicable), and to attend meetings etc hosted by other agencies.
The post-holder will have contact with body fluids ie, wound exudates; urine etc while in clinical practice.
6. Other
The postholder may be expected to work across all 8 sites and to co-operate with all staff from other practices within the network. This work will be supported by the PCN management team.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
