Job Title: Experienced Hospital Discharge Social Worker

Location: Tower Hamlets, London Borough of Tower Hamlets, United Kingdom

Rate: Up to £34 per hour

Liquid Personnel is currently recruiting an Experienced Hospital Discharge Social Worker for Tower Hamlets. The pay rate is up to £34 per hour.

Responsibilities:

The team seeks an enthusiastic individual to adopt strengths-based, person-centered assessments. Responsibilities include managing a varied caseload involving hospital discharge planning, Care Act assessments, and coordinating with healthcare professionals. You will provide exceptional social care to support adults and their carers, ensuring smooth transitions from hospital to home or other settings. You will engage in outcome-focused, preventative conversations that leverage the strengths of individuals and their networks. Collaboration with hospital staff and other agencies is essential to ensure timely and safe discharges.

Qualifications and Experience:
