Job Title: Experienced Hospital Discharge Social Worker

Rate: Up to £34 per hour

Liquid Personnel is currently recruiting an Experienced Hospital Discharge Social Worker for Tower Hamlets. The pay rate is up to £34 per hour.

Responsibilities:

Responsibilities include managing a varied caseload involving hospital discharge planning, Care Act assessments, and coordinating with healthcare professionals.
