Job summary

We are a two-site partnership with a practice population of circa 24,000 patients. We are seeking an enthusiastic Practice Nurse with Chronic Disease Management to join our friendly and forward-thinking nursing team.

The role focuses on delivering safe and effective nursing care to our patient population. The ideal candidate will be able to work independently and as part of the general practice team, demonstrating strong professional decision-making skills and clinical expertise.

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975, requiring a Disclosure and Barring Service (DBS) check for any criminal convictions.

Main duties of the job

The purpose of this role is to:

  1. Deliver nursing services and care to the practice population.
  2. Support patients in healthy living, monitor chronic conditions, and conduct health promotion and screening activities.
  3. Undertake administrative duties to support the practice's smooth operation, including clerical support to clinical staff.
  4. Facilitate communication between patients, primary healthcare team, secondary care, and other healthcare agencies.

You will work at the Registered Nurse Level as described in the Primary Care and general practice Nursing career and core capabilities framework. Experience in chronic disease management is essential. Please see the full job description for more details.
