Job description

Are you an experienced, dedicated, and passionate practice nurse ready to make an impact on patient care? Then we have the perfect opportunity for you!

Craneswater Group Practice is seeking an experienced Practice Nurse. This role is being offered through the Portsmouth South Coast PCN, which includes teams of healthcare professionals such as GPs, nurses, pharmacists, physiotherapists, and social prescribers, to provide tailored community care.

This is an exciting opportunity where no two days are the same. You will assist the medical team in caring for our patient population, including treatment, preventative care, screening, and patient education within the practice and community. As an experienced nurse, you will join our proactive team dedicated to health and wellbeing, focusing on health promotion, chronic disease management, and prevention.

This is a part-time position with negotiable hours.

Main duties of the job

The main responsibilities include:

  • Leading long-term condition management across the PCN
  • Supporting public health and screening programs, including immunizations, vaccines, and cervical screening
  • Mentoring and supervising new practice nurses and other healthcare professionals
  • Collaborating across the PCN to enhance integration with community, secondary, and social care services

Job responsibilities

The practice nurse will:

  • Assist in patient care, including treatment, prevention, screening, and education
  • Organize and coordinate nursing services in liaison with the team
  • Provide treatments independently or with GPs according to guidelines
  • Perform health screenings and manage chronic conditions such as diabetes, CHD, and asthma
  • Carry out clinical procedures like wound care, ECGs, venepuncture, and immunizations
  • Maintain equipment and stock levels
  • Collect pathology specimens and perform investigatory procedures
  • Ensure accurate documentation and administrative tasks
  • Participate in practice meetings and research projects
  • Engage in continuous professional development and mandatory training
