Job summary

North Cheshire and Mersey NHS Foundation Trust is seeking an experienced Band 5 Community Staff Nurse for the Out of Hours Community Nursing Service in Halton. The role covers evenings (17:00–23:00) with three shifts per week, including weekends, providing in-home care focusing on wound, catheter and chronic condition management.

You will work within a supportive team, ensuring continuity of care, maintaining care plans and participating in mentoring and education activities.

Qualifications

• NMC registered adult nurse.
• Evidence of level 5/6 study.
• Teaching and Assessing qualification.

Responsibilities

• Assess, plan and evaluate patients' care needs.
• Maintain up-to-date care plans in coordination with the District Nurse.
• Mentoring, teaching and assessing colleagues.
• Collaborate with health care professionals to ensure efficient service delivery.
